CenterCare Hospice and Palliative Care’s mission is to enhance the quality of life of patients  and their families by reducing or alleviating the pain inherent with a serious and chronic illness. We provide palliative care to help patients enjoy life and daily activities without unnecessary discomfort. CenterCare Hospice and Palliative Care can provide this care to anyone at any stage of illness, and can administer palliative care simultaneously with other treatments and therapies.

Typical diagnoses prompting palliative care include cancer, cardiac disease, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), neuromuscular diseases (such as Parkinson’s), Alzheimer’s disease, Amyotrophic Lateral Sclerosis (ALS), scleroderma, and renal failure.

The CenterCare Palliative team has special training and expertise in relieving pain and other symptoms caused by illness and medical treatments. Working with your doctor, we help you manage your symptoms and guide you in making a plan for living well, based on your needs, priorities and goals. When pain, trouble breathing, anxiety or other bothersome symptoms of an illness reduce comfort and quality of life for you or a loved one, CenterCare Palliative Care can ease suffering and improve quality of life for both the patient and caregiver. CenterCare Palliative Care is specialized expert medical care from a team of physicians, nurse practitioners, social workers, counselors and chaplains to support your physical, emotional and spiritual needs during serious illness.
